Take driver’s license, ID, passport, or U.S. citizenship documentation to 120 Athens Way,1-877-808-5461.
All documents must be either originals or copies certified by the issuing agency.
They do no accept photocopies or notarized copies of documents.
If you have had a TN State ID/Driver’s license before:
- Go to office: 311 7th Ave. N.
- Go down the steps and to the left to office. Hours: Mon-Fri 8:30 to 5.
- Your Tennessee mailing address
- Your driver license/ID number.
- Last 4 digits of Social Security #
- Money: Your credit or check card (no prepaids): $8- $12
If you need a TN ID for the first time, you’ll need:
- Proof of US Citizenship: A birth certificate, passport, or other valid proof of lawful permanent residence
- Primary proof of ID: An old ID/license, birth certificate, military ID, passport, marriage license (if you changed names, you need proof)
- Secondary proof of ID: Medical records, check stubs, union cards, health insurance, financial documents, etc.
- 2 proofs of residence: (letters with typed addresses mailed to your current address within last 4 months) OR and affidavit of homelessness if in shelters/on streets
- Your Social Security # or sworn affidavit
