BIRTH CERTIFICATE:
If the person was born in TN, they can go to the TN Office of Vital Records at 710 James Robertson Pkwy, Nashville, TN 37243 and complete the application onsite. There is a cost.

People born in any state can order birth certificates online from www.vitalchek.com. The website will guide you through the ordering process step by step, but here are a few things to be aware of:

• You will be asked who is ordering the birth certificate. Because that person is with you (or perhaps is on the phone but has given consent), select the option that says the owner of the birth certificate is the one ordering.
• The website automatically selects UPS shipping (which is expensive and has to be signed for) unless you click the drop-down menu and select the standard mail option available for most states.
• ​If the person does not have a phone number to use in the contact information, you may use your own if you wish and can relay any messages or the phone number of a service organization (because they very rarely contact folks via phone for questions or updates).
• Some people will have a mailbox where they would like to receive the birth certificate, often at Room in the Inn: 705 Drexel St. Nashville, TN 37203. Or some other location safe for delivery.
• If you have made arrangements with an Open Table outreach worker, it can be mailed to the OTN address: 210 Morton Ave. Nashville, TN 37211
• Some states will require that the person ordering the birth certificate send a scan or copy (front and back) of a state ID. You can take a picture or scan with a phone as long as it is very clear and doesn’t have a glare. The website will provide directions about uploading or faxing documents if necessary.
