    If you have had a TN State ID/Driver’s license before:

    1. Go to office: 311 7th Ave. N.

    2. Go down the steps and to the left to office. Hours: Mon-Fri 8:30 to 5.​

    3. Your Tennessee mailing address

    4. Your driver license/ID number.

    5. Last 4 digits of Social Security #

    6. Money: Your credit or check card (no prepaids): $8- $12


    If you need a TN ID for the first time, you’ll need:

    • ​​​Proof of US Citizenship: A birth certificate, passport, or other valid proof of lawful permanent residence

    • Primary proof of ID: An old ID/license, birth certificate, military ID, passport, marriage license (if you changed names, you need proof)

    • Secondary proof of ID: Medical records, check stubs, union cards, health insurance, financial documents, etc.

    • 2 proofs of residence: (letters with typed addresses ​mailed to your current address within last 4 months) OR and affidavit of homelessness if in shelters/on streets

    • Your Social Security # or sworn affidavit


    ​SOCIAL SECURITY CARD:
    ​Take driver’s license, ID, passport, or U.S. citizenship documentation to 120 Athens Way, 1-877-808-5461.
    All documents must be either originals or copies certified by the issuing agency.

    They do no accept photocopies or notarized copies of documents.

  • ​The Criminal Court Clerk’s office welcomes walk-ins, no appointment is necessary: 408 2nd Ave. N.

    The applicant will need a valid license or state issued ID to receive assistance with expungement questions.

  • If the applicant was born in TN, they can go to the TN Office of Vital Records at 710 James Robertson Pkwy, Nashville, TN 37243 and complete the application onsite. There is a cost.

    People born in any state can order birth certificates online from www.vitalchek.com. The website will guide you through the ordering process step by step, but here are a few things to be aware of:

    • You will be asked who is ordering the birth certificate. Because that person is with you (or perhaps is on the phone but has given consent), select the option that says the owner of the birth certificate is the one ordering.

    • The website automatically selects UPS shipping (which is expensive and has to be signed for) unless you click the drop-down menu and select the standard mail option available for most states.

    • ​If the person does not have a phone number to use in the contact information, you may use your own if you wish and can relay any messages or the phone number of a service organization (because they very rarely contact folks via phone for questions or updates).

    • Some people will have a mailbox where they would like to receive the birth certificate, often at Room in the Inn: 705 Drexel St. Nashville, TN 37203. Or some other location safe for delivery.

    • If you have made arrangements with an Open Table outreach worker, it can be mailed to the OTN address: 210 Morton Ave. Nashville, TN 37211

    • Some states will require that the person ordering the birth certificate send a scan or copy (front and back) of a state ID. You can take a picture or scan with a phone as long as it is very clear and doesn’t have a glare. The website will provide directions about uploading or faxing documents if necessary.
